If you are requesting a new fit note you should speak with a Health Professional in the first instance.
If you have a request for an ongoing fit note you may be able by speaking with a member of our reception team. In some instances the GP may need to speak with you before issuing a fit note. Should your fit note be granted you can collect this from our Reception Desk from the 5th working day following request.
