How To Register

Our reception staff are happy to help you register with the Practice, however, you have to be staying permanently in the Practice area (Bathgate and Torphichen).  Patients currently registered with us residing in Whitburn, Blackburn, Armadale, Livingston or Westfield will remain registered with the practice – however, if you move address within or to any of these areas, you will be asked to register with a more local practice.  This is to ensure that the practice can provide fast and appropriate emergency care to a patient’s that falls ill suddenly.

You will be asked to complete the registration form and new patient questionnaire (found below), please complete and return to the practice. If you have no access to a printer then please collect registration forms from our Reception Desk.  We will accept registration forms via the post to save unnecessary footfall to the practice.  If you are requiring any medication which was on a repeat prescription issued in the last two months please the New Patient Medication Form for this to be passed to GP for consideration.

In Bathgate Primary Care Centre, Kingsgate Medical Practice is one of three medical practices.  We share registration of patients between the three practices so please check with the practice which surgery is currently registering patients prior to posting in registration forms.
